How Hormones Trigger Migraines in Women

Estrogen and progesterone, two key reproductive hormones, naturally rise and fall throughout the menstrual cycle. In the follicular phase (the first half of the cycle), estrogen gradually rises, peaking just before ovulation. After ovulation, during the luteal phase, progesterone increases while estrogen begins to drop. This drop in estrogen, especially if it’s steep or sudden, and/or insufficient progesterone levels can trigger a cascade of symptoms, including intense migraines.

I see this pattern often in my practice. Many women who suffer from recurrent premenstrual migraines respond extremely well to bioidentical progesterone support during the luteal phase, typically from day 15 or 18 to 25 of their cycle. For those who experience migraines at the onset of their period, using a small dose of transdermal estradiol from days 1 to 4 can help stabilize estrogen levels and dramatically reduce symptoms.
